To access service:
- contact the department for a donor registration form and a donor card
People wishing to donate their body may:
- include a statement in their wills, however, this will not ensure that it is followed through because a will may be read after the burial or cremation
A body is not accepted without consent from one of the following:
- next-of-kin
- executor
- person lawfully in charge of your body at the time of death
Completion of the form does not guarantee acceptance of donation.
