This short term program provides frequent at home support to children, youth and families with the goals of helping manage mental health and substance use concerns, navigate community systems and develop a plan for connection to a longer term support team
A Mental Health Nurse will meet with clients in their home or in the community upon referral from another professional who has identified that the needs are urgent in nature. Within 4 business days of referral, contact with the family will be made and it will be determined what can be done to best support their needs. We will support urgent or chronic needs in the areas of mental health, substance use or parenting. Support for the current concerns as well as navigation of community systems and connection to the appropriate longer term resources will be provided by a multidisciplinary team.
