Provides medical care and case management of the most medically fragile children in Alberta Children Hospital's complex care catchment. Coordination of care during inpatient admissions and as patients transition from a prolonged hospital stay(s) back into the community.
The Alberta Children's Hospital Complex Care Program provides inpatient and ambulatory care and system navigation until care is fully transferred to a community pediatrician and medical home. For patients frequently re-admitted, the CCP serves as the primary medical home until stability allows for community transfer. The goal is to reduce hospital admissions, shorten stays, and improve family self-efficacy by ensuring consistent healthcare providers familiar with the patient’s history, facilitating care, knowledge translation, and comprehensive coordination across specialties.
