Service provides:

• outreach support for older adults, including identifying participant needs and goals and facilitating connections to community resources, services, and information to help participants reach their personal definition of wellbeing

Includes:  

• assessment of needs and risk factors
• filling out forms
• finding appropriate housing, food, or transportation options 
• assistance navigating government benefits, including Old Age Security (OAS), Canada Pension Plan (CPP), Guaranteed Income Supplement (GIS), Alberta Seniors Benefit, Dental and Optical Assistance for Seniors, and Assured Income for the Severely Handicapped (AISH)
• exploring educational and recreation programs
• connecting with healthcare professionals when needed
• finding reputable service providers for snow removal, yard maintenance, light housekeeping, and personal services
• offering resources around decision-making tools
• case management, including helping participants cope with complex issues (grief, financial difficulties, mental health and emotional well-being, elder abuse, substance use, and social isolation) by connecting them to service providers
• in-person visits, which may include home visits, community visits, and accompanying clients to appointments (location and type of visit vary depending on the partner organization, capacity of workers, and presenting needs of the participant)
