Service offers:
- a variety of community services
Includes:
- Housing First Centralized Intake - an intake worker will sit with applicants and complete a screening interview to determine suitability for program
- Housing First Case Management - a case worker provides 12 months of intensive case management to chronically homeless Indigenous peoples which involves working together to address barriers to attaining / maintaining safe, stable housing and working on goals of self-sufficiency and autonomy
- Rapid Rehousing - a caseworker provides 2 to 6 months of intensive case management to those who are experiencing episodic or transitional homelessness to get them into housing and out of shelters as quickly as possible
- Hospital / Justice Liasion - a liasion worker assists clients who are being discharged from medical or justice settings
- Outreach - focuses on providing one-on-one services to individuals or families either homeless or at risk of homelessness that do not qualify for either the Rapid Rehousing or Housing First programs
