Service targeted to:
- a client with the Cerebral Palsy Association of Alberta for at least 3 months
- individuals with disabilities
Referrals made by doctors, therapists, social workers, or audiologists:
- letter of support states the applicant's need for funding and how it will impact their quality of life
- statement of diagnosis indicates that the applicant is diagnosed with a disability
