Seniors Community Services Partnership: Seniors System Navigation Team
Provides referrals and service navigation, case management, or outreach support to seniors 55 years and older or caregivers in Lethbridge, Lethbridge County, Cardston, Cardston County, County of Warner, Taber, MD of Taber, Pincher Creek and Crowsnest Pass.
Service offers:
- senior system navigation, information and referral service
- assistance with applying for financial benefits / assistance and housing
- counselling session
- short term case management related to aging challenges
- educational and support groups
- outreach support
