Referrals to social work, physiotherapy, occupational therapy, speech-language pathology and spiritual care will only be considered for a time-limited short service to connect the patient with community services and resources.
The purpose of this clinic is to review patient concerns, provide short team interventions and help connect patients to community resources, including education on self-care.
The typical sessions offered are in the range of 1 to 3 visits. The service is provided predominantly by virtual means such as phone calls, Zoom meetings, and electronic communications. In-person appointments will be provided for specific and skilled interventions when identified by the service provider.
This clinic is unsuitable for patients requiring detailed assessment and follow-up treatments, and for which refer the patient to other suitable ambulatory clinics and community resources. No inter/multi-disciplinary services are available through this clinic.
