Provides support to Glenrose Rehabilitation Hospital pediatric patients and their caregivers as they transition from pediatric care to the adult health care system.
Service inludes:
helping to navigate the pediatric to adult healthcare transition with supports, tools and resources
help addressing concerns of clients, families, and members of the healthcare team
information and education (e.g., making good decisions, caregiver stress, career or work options, child care options)
referrals to community resources and services (e.g., financial help, accommodation, transportation)
