Transitions to Independence Program

Provides flexible and responsive transitions for youth with child intervention status to adult services and / or independence.
Service provides:
flexible and responsive transitions for youth with child intervention status from residential care, foster care, a parental home, incarceration, hospitalization, or homelessness to adult services and / or independence
support for youth who may lack sufficient family support and / or community resources to stabilize the youth in independent living or have various presenting issues
May include:
obtaining semi-supported or independent housing
living and life skills training
meaningful daily activity
family and natural connections
transition, support, and after-care planning
mentorship and cultural opportunities
referrals to community resources
