Assists vulnerable individuals in defining meaningful goals and creating action plans, and connects them to a wide range of programs and resources to achieve these goals.
Service provides:
individualized assistance and needs-based planning to define meaningful goals, create action plans, and access a wide range of supports
Includes:
life skills support, including activities of daily living (ADLs)
help accessing basic needs, such as food and clothing
help accessing financial and income support, such as Assured Income for the Severely Handicapped (AISH) and Alberta Aids to Daily Living (AADL)
employment-related supports and referrals, such as resume creation, interview skills, and job searching
help addressing and navigating educational goals and needs, including education support for family members
help acquiring official identification, such as driver's licence, health card, and social insurance number (SIN)
support locating and applying for housing
help accessing health supports, such as locating physicians and other health professionals
referrals for mental health and addictions support
advocacy
cultural connections support and referrals

HOURS OF OPERATION
ELIGIBILITY
Service targeted to:
18 years and older
living in the Wood Buffalo region
who are willing to engage in goal setting and action planning
including individuals with vulnerabilities and complex social needs
Individuals must
not:
be in active crisis
be in active addiction
require mental health or addictions support requiring long-term (greater than 12 weeks) support or specialized treatment / intervention
have an active, open, unresolved file in the justice system
