The Neonatal Transition Team (NTT) is a community-based program providing support for the family following infant discharge from the Neonatal Intensive Care Unit (NICU) and / or Pediatric Units.
Services are provided in home, in clinic or virtually. Supports include the following:
- information about the infant's health, abilities, and special needs
- regular assessments of the infant's health, growth, and development
- support for breast, bottle and nasogastric tube feeding
- guidance to enhance infant nutrition
- reflux management
- home oxygen therapy and respiratory assessment
- blood pressure monitoring as required
- liaison with the infant's pediatrician/PCP and specialists such as registered dieticians and occupational therapists
- referrals to other services and community resources such as social workers, lactation consultants and physiotherapists
NTT will support family as needed to a maximum of 16 weeks corrected age (~3-4 months post NICU discharge).
