The Street Connect team provides both outreach and in-reach services, trying to meet clients where they are, to eliminate any barriers to accessing services. The mission of the Street Connect program is to provide client centered care to the unhoused population with high severity complex mental health needs within the City of Red Deer. The program will extend the continuum of addictions, mental health, and physical health service options for unhoused individuals with high severity complex mental health needs. The program entails a primary and secondary stream of service:

• Primary Service (Wraparound care): clients who are appropriate for programming will work with the multidisciplinary team developing a treatment plan, goal setting, and working with clients for their mental health, addictions, and medical needs
• provide community resources and link to more appropriate services through a warm handoff

The multidisciplinary team of Street Connect provides assertive outreach and in-reach services to homeless clients who are ineligible for other services.
