Seniors Mental Health and Addiction Response Team
Provides seniors with access to mental health and addiction services within the community. These services are non-denominational and open to individuals of all faiths, cultures, and backgrounds.
Service offers:
- outreach support from an interdisciplinary team consisting of social workers and licensed practical nurses
- focus on prevention and early intervention
- screening and assessments
- ongoing mental health monitoring
- addressing basic medical needs
- action plan implementation
- advocacy on the needs and specialized resources required to support seniors
