Work as part of a multidisciplinary team providing social work assessment, intervention and consultation for inpatients, outpatients, emergency department patients and long term care residents.
Services provided include (but are not limited to):

- domestic violence
- sexual assault support
- family dynamics
- advance care planning
- placement / transition planning
- caregiver support
- liaison with other programs
