Wellness Integrated Support Homes (WISH) is a long-term supportive housing program for adults experiencing addiction and mental health issues.
Offers living spaces for adults who need 24 / 7 access to help with daily activities.
Services include:
- help with daily activities, home making, and medicines
- monitoring health
- access to recreation activities
- scheduling professional services
Living spaces are usually small group homes where 5 to 8 people live together and get support from a Health Care Aid. Residents have private bedrooms and share all other common areas of the home. Basic bedroom furnishings are provided.
Each resident has a Case Manager who assesses health care needs on an ongoing basis, co-ordinates and schedule access to health care services.
