Provides case management for individuals with an acquired brain injury.
Service offers:
- case management and assistance to clients with goal setting and reaching those goals
Includes:
- assessment
- support, advocacy, and referral to community-based supports
- building and surveying community capacity for service to brain injury survivors
- offering lifetime services as needed and long term follow-up services, including the intake process for Supports for Community Living
- occupational therapy
- electronic compensatory device training
Also provides access to other services that may help meet a client's goals:
- Community Living and Rehabilitation - assists clients and families with managing cognitive changes after brain injury, with a focus on enabling participation in the community and independence in life, as well as occupational therapy services, Vocational Assessment and Return to Work support
- Computer Assistive Device Training - offers a small classroom setting where participants can learn a variety of computer skills, such as internet and e-mail, Microsoft Office, image editing, and basic computer, smart phone, and tablet skills
- Life Skills Classes - offers classes and workshops on life skills such as self-esteem, effective communication and social skills, attention and memory, personal wellness, time management, stress management, and understanding brain injury
- Concussion Management Program - offers a combination of supportive counselling, occupational therapy, quality of life assessment, and referrals to relevant medical care providers for physical symptom management
