Provides residential support to individuals requiring assistance to live in the community.
Service provides:
- low income, secure accommodations for individuals with varying levels of care requirements and special needs such as physical disabilities, challenging behaviours, hearing loss, or visual impairments, with 24 hour supports including personal care, social, and recreational activities
- assisted living accommodations, shared residence, or rented suites from a support person for those who do not require continued access to professional services
Includes:
- community-based recreational and community access supports that assist individuals with disabilities in accessing the community by overcoming individual and environmental barriers to community integration
- daily household routines
- personal care / personal hygiene routines
- education
- instruction and support in communication and American Sign Language (ASL)
- life skills and daily living independence skills
- social and leisure activities
Residents are peer matched according to age, disability, supports required, culture, and funding source. Sites available for those with dementia who are fearful or at risk of wandering.
